Job Summary

We are seeking a skilled and empathetic Registered Nurse Case Manager to join our team. As a Hospice Registered Nurse / RN Case Manager, you will play a vital role in providing comfort, peace, and dignity to our patients and their loved ones.

Key Responsibilities

  • Enable patients to spend quality time with their loved ones and engage in activities they enjoy
  • Build trusting and compassionate relationships with patients and their families
  • Empower caregivers by educating them on care for their loved ones
  • Collaborate with the care team to provide comfort, peace, and dignity

Requirements

  • RN license in the state you work
  • Two years of experience as a registered nurse, hospice experience preferred
  • Current driver's license
  • Current CPR and BCLS certification
