Position Overview
As a Registered Nurse, you will conduct thorough assessments of residents' health and educational needs upon admission and throughout their stay. You will maintain awareness of any significant changes in their condition and communicate these to the appropriate interdisciplinary team (IDT) members. Accurate documentation of all assessments is essential. You will create dynamic care plans tailored to the residents' identified needs, collaborating with physicians, residents, families, and other IDT members. Regular reviews and updates of the care plan will be necessary as residents' needs evolve. You will ensure that the care plan is communicated effectively to all relevant IDT members, residents, and families, documenting any changes as required.
Your role will include identifying necessary nursing interventions related to the care plan, which may involve medication administration, treatments, and IV fluid management, all performed safely and promptly. You will document all interventions accurately and in a timely manner. Critical thinking skills will be vital when addressing changes in residents' conditions, and you will notify physicians promptly using the SBARR communication tool. You will maintain professionalism during crisis situations, such as family concerns or staff disputes, ensuring calm and effective resolution.
Monitoring residents' care closely will be part of your responsibilities, ensuring that the care plan is implemented effectively. You will identify areas needing improvement and actively engage in the QAPI process for immediate resolution. Staying informed about residents' statuses will help you quickly recognize any behavioral changes or concerning symptoms. Assisting with resident care, including activities of daily living (ADLs), will also be part of your duties. You are expected to complete a minimum of twelve hours of nursing-related training annually and remain committed to best practices in nursing through ongoing education and self-study. Additional responsibilities may be assigned as necessary.
Qualifications
- Graduation from an accredited nursing program resulting in RN licensure
- Current RN license in the relevant state
- CPR/AED certification required or willingness to obtain within six months
- Preferred experience in long-term care settings
- Strong critical thinking abilities
- Excellent verbal communication skills for interaction with patients, visitors, and staff
- Outstanding time management and organizational capabilities
- Ability to stand and walk for extended periods
- Capacity to lift heavy objects using proper techniques and assistance as needed
- Ability to transfer and assist patients weighing over 200 lbs with appropriate techniques
- Capability to push and pull equipment exceeding 40 lbs using proper methods
- Manual dexterity and coordination when working with patients and medical equipment
- Visual acuity to distinguish colors
- Ability to perform necessary documentation both electronically and in writing
