Job Description:

Overview:

Under the leadership of the department Director, Assistant Directors, and/or Pharmacy Supervisors, the Clinical Pharmacist is responsible for planning, directing, assessing, and providing pharmaceutical care according to the policies and procedures of the hospital and department.

Key Responsibilities:

  • Manage pharmacy protocols and provide inpatient medication order screening and verification
  • Provide patient consultations and complete job functions and documentation timely
  • Assess and manage patient medication regimens, including order processing, sterile compounding, and therapeutic drug monitoring
  • Prescribe medications per drug protocols and provide drug information
  • Identify and report adverse drug reactions and medication errors, and provide patient consultation
  • Develop plans of care and oversee Pharmacy Technicians
  • Respond to emergency situations, including codes

Requirements:

  • State of California RPH (Pharmacist) license is required
  • American Heart Association Healthcare Provider BLS is required
  • ACLS must be completed prior to completion of the introductory period and independent assignment
  • Board certification preferred
  • DEA license required and must be completed at the end of the introductory period
