Position Overview
The Clinical Specialty Care Pharmacist plays a vital role in delivering specialized pharmacy services within a clinical setting. This position is integral to the multidisciplinary care team, ensuring optimal medication management and patient outcomes.
Key Responsibilities
- Provide specialized pharmacy care services across various clinical sites.
- Collaborate effectively with healthcare professionals, including physicians and nurses, to enhance patient care.
- Conduct comprehensive patient assessments, including medication history and laboratory evaluations.
- Manage medication therapy and disease states, adjusting therapies as necessary to optimize patient outcomes.
- Ensure the seamless flow of prescriptions through the specialty pharmacy, addressing any financial barriers to medication access.
- Maintain accurate data for reporting and analysis, contributing to continuous quality improvement initiatives.
- Educate medical staff and patients on medication use and safety.
Qualifications
Applicants must possess a valid pharmacist license and a Bachelor’s or advanced degree in Pharmacy. Additional certifications, such as Board Certified Specialty Pharmacist, are preferred. A minimum of two years of experience in ambulatory care or specialty pharmacy is highly desirable.
