Salary and Benefits

The SRO Residential Services Coordinator 3 position comes with a competitive salary range of $23-$23.60 per hour, with an additional $1 per hour shift differential for evenings and overnights. Job Description

The SRO Residential Services Coordinator 3 provides direct support, advocacy, and education to residents in the program. This includes assisting with the development of individualized service plans and coordinating services and functions to enhance residents' autonomy, quality of life, and recovery process. The role involves working collaboratively with the resident's health home care manager and other community service providers to ensure continuity of care.

Responsibilities

  • Conduct tours and lunch visits with prospective residents and provide input to the Admission/Management team regarding acceptance into the program.
  • Ensure comprehensive service planning in a person-centered approach during the resident's tenure with the program.
  • Conduct quarterly service plan reviews with the resident and their support team to assess progress and make revisions as needed.
  • Provide teaching, monitoring, and recovery-oriented supports to residents, including rehabilitative services such as assertiveness/self-advocacy training, community integration/resource development, daily living skills training, and medication management.
  • Maintain accurate and timely resident files, including documentation of progress notes, updating of service plans, and maintenance of psychiatric and medical records.

Qualifications

To be successful in this role, you will need to meet one of the following criteria:

A. Bachelor's degree in a Human Services field plus at least one year of experience working in a Human Services field.

B. Associate's degree in a Human Services field plus at least three years of related experience working in a Human Services field.

C. High School diploma/GED plus at least five years of experience working in a Human Services field.

This position requires a clean/valid New York State driving record as outlined in DePaul's personnel policy. You must be at least 18 years of age.

Work Environment

The schedule for this position is Tuesday-Saturday, 3-11pm. We value a supportive work environment and offer opportunities for professional development and career advancement.
