About the Role:
The Evergreen Crossing Primary Care Clinic is dedicated to delivering essential healthcare services to individuals facing homelessness and other challenges. As a vital part of Central City Concern's Recuperative Care Program (RCP), this role involves providing immediate medical support and case management to a diverse clientele.
Key Responsibilities:
- Deliver comprehensive primary, urgent, and acute medical care to low-income adults with various health conditions.
- Collaborate with a multi-disciplinary team to ensure holistic care and support for patients.
- Participate in Medication Supported Recovery services and support patients undergoing Hepatitis C treatment.
- Conduct thorough health assessments, including histories and physical examinations.
- Manage chronic illnesses and provide care for acute medical issues.
- Prescribe medications and implement treatment plans tailored to individual patient needs.
- Engage in continuous improvement initiatives and contribute to quality assurance activities.
Qualifications:
Applicants should possess a strong background in primary care, mental health, and addiction medicine. Experience working with underserved populations is highly valued. Candidates must hold appropriate licensure to practice in Oregon and meet the training requirements for buprenorphine prescribing.
