Job Summary

We are seeking a dedicated and compassionate RN Case Manager PRN to join our team at HCA Florida Capital Hospital. As a key member of our rehabilitation team, you will play a vital role in ensuring high-quality, patient-centered care for our patients.

Key Responsibilities
  • Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences to the patient, family, and caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
  • Registered Nurse (RN) or Advance Practice Registered Nurse (APRN) degree
  • Associate Degree or Registered Nurse Diploma
