Case Management Professional

We are seeking a dedicated and compassionate Case Management Professional to join our team at The Villa Outpatient Services LLC. As a key member of our recovery services, you will work closely with clients to develop and implement personalized treatment plans.
Job Description:
- Manage a caseload of clients, working directly with them to address legal, medical, financial needs, and provide individual and group therapy.
- Develop and maintain client treatment records and reports, ensuring confidentiality and accuracy.
- Collaborate with other staff members to perform clinical assessments and attend treatment team meetings.
Key Responsibilities:
- Provide counseling and guidance to clients, assisting them in overcoming dependencies, maladjustments, and relapse prevention.
- Develop and implement aftercare plans, beginning as soon as possible upon admission.
- Coordinate with clients and individual therapists to identify additional needed support services, including educational, vocational, legal, recreational, mental health, and social services.
Requirements:
- Minimum of a High School Diploma or GED; Bachelors Degree preferred.
- Licensure: Maintains current registration, certification, or license from a DHCS-approved certifying organization.
- Experience: 2 years of previous chemical dependency treatment experience in the addiction field preferred.
- Medical Requirements: Must have a TB screen, Health Screening, CPR/First Aid Certificate within 7 days of hire.
Compensation and Benefits:
