Clinical Pharmacist

Mission Box Solutions is seeking a dedicated and experienced Pharmacist to join their team. As a key member of our healthcare team, you will play a vital role in ensuring safe, cost-effective, and appropriate medication use.
Key Responsibilities:- Prepare and dispense medications per physician orders, adhering to established policies, procedures, and protocols.
- Accurately interpret and transcribe drug orders into the patient medication record, ensuring accuracy and attention to detail.
- Compound and dispense sterile and non-sterile pharmaceuticals, including chemotherapy and parenteral nutrition, with precision and care.
- Manage and document controlled substances in compliance with legal and institutional policies, maintaining confidentiality and security.
- Monitor drug therapy for safety, contraindications, and interactions, providing clinical consultation to healthcare providers and patients.
- Offer drug education to patients and their families, supporting optimal medication use and adherence, and promoting healthy behaviors.
- Participate in clinical interventions, including IV to PO conversions, renal dosing, and pharmacokinetic support, to optimize patient outcomes.
- Supervise and direct pharmacy technicians, ensuring the accuracy of their work and promoting a culture of excellence.
- Engage in interdisciplinary rounds and collaborate with healthcare teams to improve patient care and outcomes, fostering a culture of teamwork and communication.
- Maintain current pharmacist licensure and complete all required continuing education, staying up-to-date with the latest developments in the field.
- Ensure compliance with all competency and skill assessments as required by the department, upholding the highest standards of professionalism.
- Doctor of Pharmacy (PharmD) or BS in Pharmacy.
- Hospital or acute care experience required.
- Pharmacy registration in the state of practice with state licensure within 60 days of start date.
- BLS or ACLS and PALS (pediatric practice) certifications within 180 days of start date.
