Advanced Healthcare Professional for Home Care

We are seeking an experienced Physical Therapist to join our team at LHC Group in Worcester, Massachusetts. As a key member of our home health care team, you will play a vital role in helping patients recover from injuries or illnesses and maintain their independence in the comfort of their own homes.
About the Role:The successful candidate will be responsible for assessing and evaluating patient care needs, developing and implementing personalized treatment plans, and collaborating with other healthcare professionals to ensure comprehensive care. With a strong focus on patient-centered care, we prioritize building trusting relationships with our patients and providing them with the support and guidance they need to achieve their goals.
Key Responsibilities:
- Evaluate patient functional status and physical therapy needs, working closely with physicians to develop effective treatment plans.
- Develop and implement individualized treatment plans, using evidence-based practices to promote optimal patient outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care, ensuring seamless transitions between healthcare settings.
- Communicate effectively with patients, families, and caregivers, educating them on treatment options, progress, and goals.
Requirements:
- Current licensure as a Physical Therapist in the state of MA.
- Active CPR certification.
- Valid driver's license and access to a reliable vehicle.
