The Patient Care Resource Manager is a key role at the Ohio State University Wexner Medical Center, responsible for facilitating patients through the healthcare system. This position involves pre-admission through post-discharge coordination of care, resources, and services, utilizing the nursing process to enhance patient care decisions and throughput.
Key Responsibilities
- Monitor and coordinate patient plans of care to ensure timely and efficient care delivery
- Collaborate with physicians, nurses, and other healthcare workers to manage patient transitions to the next level of care
- Communicate with third-party payers to gain approval for post-discharge care
- Assist physicians in ensuring the appropriate level of care for each patient using evidence-based criteria
Requirements
- Bachelor's degree in Nursing
- RN licensure
- Five years of recent clinical experience applicable to the population
- Knowledge of clinical practice guidelines, computer databases, and payer protocols
