Key Responsibilities- Conduct comprehensive assessments of patients and develop individualized care plans to address their unique needs and goals.
- Provide high-quality, patient-centered care to patients in their homes, including routine check-ups, medication management, and disease management.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure seamless care coordination and communication.
- Stay up-to-date on the latest medical research, technologies, and best practices to provide the highest level of care to our patients.
- Participate in ongoing quality improvement initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery.
- Current state RN license and NP license or ability to obtain before start date.
- Completed Nurse Practitioner accredited program before start date.
- Access to reliable transportation.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
