Job Summary

We are seeking a highly skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist at Concept Rehab, you will be responsible for evaluating, treating, and supervising patients with communication, problem-solving, cognitive processing, and swallowing difficulties. You will work closely with families, physicians, and other healthcare team members to provide comprehensive care and maintain accurate documentation of services.

Key Responsibilities
  • Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
  • Develop effective treatment plans that meet the unique needs of each patient.
  • Treat patients according to the treatment plan of care, utilizing evidence-based practices and techniques.
  • Communicate with physicians, clinical managers, and other healthcare team members to ensure seamless care coordination.
  • Participate in care conference meetings and interdisciplinary team meetings to discuss patient progress and develop comprehensive care plans.
  • Provide in-service training to facility staff on restorative and functional maintenance programs.
  • Record evaluations, treatment notes, progress notes, and discharge summaries in a timely and accurate manner.
  • Instruct patient families or nursing staff in maintenance programs to promote continued care and support.
  • Participate in discharge planning to ensure a smooth transition for patients.
  • Achieve or exceed minimum productivity standards established by Concept Rehab.
  • Recommend equipment to facilitate improved delivery of clinical services.
  • Report any problems with department equipment to appropriate personnel.
  • Act as a clinical supervisor for affiliating Speech Language Pathology graduate clinicians.
  • Maintain an organized working environment and consistently portray the mission, vision, core values, and professional image of Concept Rehab.
Requirements
  • Master's Degree in Speech Language Pathology.
  • Certified by the American Speech-Language-Hearing Association.
  • Licensed in the state(s) where services are provided to practice as a Speech Language Pathologist.
