**Job Summary:**

The Mary T Family of Companies is seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will be responsible for evaluating the needs of referred patients, developing treatment plans, and providing physical therapy services in a home care environment.

**Responsibilities:**

* Evaluate patient needs and develop individualized treatment plans to improve mobility, reduce pain, and restore function
* Implement physical therapy interventions, including exercises, training in assistive devices, and instruction on daily living activities
* Document patient care and progress
* Supervise physical therapy assistants
* Collaborate with RN case managers, physicians, and other healthcare professionals to ensure comprehensive care

**Requirements:**

* Master's Degree in Physical Therapy
* Current Physical Therapy License in Minnesota
* Valid driver's license
* Minimum one year experience in home health, short-term rehab, outpatient, or hospital inpatient rehab setting
