Case Manager

We are seeking a highly motivated and experienced Case Manager/Intake Specialist to join our team at Endeavors. As a key member of our workforce wellness program, you will play a critical role in promoting recovery and wellness, community integration, and independent living for Customs/Border Patrol (CBP) employees and their families.
Key Responsibilities- Advocate for and facilitate clients' access to wellness services and community resources, building a database of community resources and providing navigation to referrals.
- Provide comprehensive case management to CBP employees and their families, including behavioral health and wellness services, and prepare and maintain confidential case records.
- Conduct outreach and networking activities in the community, providing presentations at various community groups and promoting the success and reputation of the CBP Workforce Wellness Program.
- Develop a network of trusted contacts within organizations to refer and coordinate care, and consult with service providers and community partners on resource-related issues.
- Complete initial eligibility screenings of potential clients, provide general information and community referrals to clients not meeting program eligibility requirements, and complete intake and case management for eligible clients.
- Bachelor's degree in social work, sociology, psychology, or a related field, or any combination of experience and education that results in the required knowledge, skill, and abilities.
- 1-3 years of case management experience, with 2+ years in a customer service-focused environment preferred.
- Experience with law enforcement, veterans, and their families is a strong plus.
- Driver's license with a clear record, and LMSW, LBSW, or LMFT preferred.
