Pediatric and Adult Patient Caregiver

About the Position:
We are seeking a caring and dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care and services to pediatric and adult patients in their own homes. As a member of our team, you will have the opportunity to make a real difference in the lives of our patients and their families.
Key Responsibilities:
This position involves providing on-going assessments and evaluations of patient care, collecting and analyzing data to identify potential nursing diagnoses, and implementing interventions to improve patient outcomes. You will also be responsible for communicating effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Requirements:
To be considered for this role, you must have current licensure as an LPN/LVN in the state in which you will practice, as well as one year of prior professional nursing experience. A current TB or Chest X-Ray and BLS card are also required.
