About the Role

We are seeking a skilled Registered Nurse to join our team as a Care Manager RN Per Diem/Evening. As a Care Manager, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments and identify patient needs, developing strategies to address social determinants of health.
  • Facilitate communication and care coordination across the continuum, ensuring effective transitional care management.
  • Provide education and support to patients and families, empowering them to make informed decisions about their care.
  • Participate in quality improvement initiatives, contributing to the development of best practices and policies.
Requirements
  • Bachelor's Degree in Nursing or equivalent education/experience.
  • California Registered Nurse License upon hire.
  • 2 years experience in Healthcare related field (Acute, Ambulatory, Post-Acute, etc.).
  • 2 years experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • Master's Degree in Healthcare related field.
  • Certification in Case Management (ACM or CCM) upon hire.
