About the Role

As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and contributing to our hospital's mission to maximize the health, function, and quality of life of those we serve.

Responsibilities
  • Accurately perform patient assessments and identify patient needs to provide personalized care.
  • Initiate appropriate nursing interventions to ensure timely and effective treatment.
  • Provide care that is tailored to the patient's condition and age, promoting optimal health outcomes.
  • Document medical necessity and patient progress in a timely and accurate manner.
  • Develop and revise Interdisciplinary Care Plans for each patient, ensuring comprehensive and coordinated care.
  • Conduct quality improvement assessments to drive continuous improvement in patient care.
Qualifications
  • Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
  • Current state licensure as a Registered Nurse.
  • CPR/BCLS certification, with ACLS preferred.
  • CRRN certification is a plus.
  • Minimum six months of Medical/Surgical experience in an acute care setting, with a strong preference for experience in rehabilitation settings.
  • Excellent oral and written communication, as well as interpersonal skills, to effectively collaborate with patients, families, and healthcare teams.
