Comprehensive Care Nurse Practitioner

Key Responsibilities:
- Acts as the central coordinator of the Care Team, delivering ongoing support throughout the patient care continuum, formulating personalized care strategies, and evaluating patient outcomes.
- Provides specialized knowledge in general healthcare and/or evidence-based mental health services to adult patients with chronic and intricate health issues, both in-person and via telehealth, collaborating with the internal team for escalations as necessary.
- Conducts Initial Assessment visits with patients, utilizing assessment tools while engaging with other interdisciplinary team members as needed.
- Orders, interprets, and evaluates diagnostic tests, recommending follow-up consultations and additional treatment options as required.
- Prescribes and oversees medications and treatments as appropriate, including the administration of injectables.
- Documents diagnosis details, including billing codes, and processes claims.
- Identifies social determinants and non-medical obstacles to care for patients, collaborating with the Care Team to address these issues.
- Manages the psychosocial aspects of each patient's care.
- Engages actively with interdisciplinary team members through regular patient rounds to enhance care planning efficiency.
- Initiates internal referrals to leverage additional expertise within the Quartet Care Team.
- Builds strong relationships with a diverse patient population (considering socioeconomic status, age, sexual orientation, etc.) both in-person and through telehealth, fostering patient engagement in the program.
- Maintains high standards of clinical customer service by delivering compassionate, high-quality care.
- Documents all clinical findings and services provided in the electronic health record in accordance with compliance standards.
Essential Qualifications:
- Current nurse practitioner license and certification, in good standing with the state of New Jersey Board of Nursing.
- Expertise in diagnosing and treating patients with chronic and urgent medical and psychological conditions.
- Ability to travel locally at least 75% of the time to provide care in home and community settings.
- Possession of a valid driver's license, active auto insurance, access to a vehicle, and willingness to travel to patient locations.
- Exceptional written and verbal communication skills, emphasizing clarity and empathy.
- Proficient computer skills, including experience with Electronic Health Records (EHR).
- Consistently exemplifies Quartet's values, including strong teamwork, patient-centered high-quality care, innovation, excellence, and productivity.
- Maintains high levels of patient satisfaction.
- Willingness to participate in after-hours call coverage.
Preferred Qualifications:
- Experience in home-based care, complex care management, or working with patients with co-occurring substance use disorders.
- Familiarity with value-based care models.
- Previous experience in telehealth.
- Skilled in utilizing online tools and technology for care delivery and patient communication.
- Dual FNP/PMHNP qualifications.
- Highly collaborative and effective in a team environment.
- Bilingual candidates (Spanish and/or other languages) are encouraged to apply.
- Comfortable providing evidence-based care while being accountable for clinical outcomes and quality.
- Thrives in a start-up environment, contributing to the establishment of innovative care models.
