MICA LPN

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Phaxis, an adult family shelter serving clients in a former hotel setting.

The LPN will play a critical role in triaging the most medically needy clients and connecting them with appropriate care. This position requires prior experience working with MICA patients and in homeless shelter settings.

Key Responsibilities:

• Triage and assess the medical needs of clients
• Develop and implement care plans for clients
• Collaborate with healthcare teams to ensure seamless care
• Provide education and support to clients and their families

Requirements:

• NYS LPN licensure
• Prior experience working with MICA patients
• Prior experience working in homeless shelter settings
