Physical Therapist Assistant Home Health

We are seeking a skilled and compassionate Physical Therapy Assistant to join our Home Health team. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Implement care plans and communicate with patients, families, and healthcare professionals to ensure seamless care delivery.
- Provide education and training to patients and their families on basic home safety and medical equipment safety.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans.
Requirements:
- Valid license, registration, or permit to practice in the state of agency operation.
- Associates degree from an accredited school of Physical Therapy.
- Valid CPR certification.
- Ability to travel to all business locations.
