Job Description

This role involves providing direct patient care, including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
  • Coordinate patient care, including delegation of appropriate tasks to direct patient care staff.
  • Perform ongoing analysis of patient data with each patient visit and document in the patient medical record.
  • Adjust or modify the treatment plan as indicated and notify supervisor as needed.
  • Provide initial and ongoing education to patients and families.
  • Administer medications as prescribed or in accordance with approved algorithms, and document appropriate medical justification and effectiveness.
Requirements and Qualifications

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited institution.
  • Current licensure as a Registered Nurse (RN) in the state where employed.
  • CPR BLS certification.
  • Previous experience in chronic/acute hemodialysis is preferred but not required.
