We are seeking a skilled Social Worker to join our team at Mount Sinai Morningside. As a Social Worker, you will play a vital role in assessing and evaluating patients' social, psychosocial, and environmental situations to determine their needs.
Responsibilities:- Develop options and treatment plans in a timely manner
- Provide direct service to patients and families, selecting treatment modalities and casework techniques compatible with assessments and identified goals
- Identify, select, access, and make referrals to appropriate community resources
- Monitor, coordinate, and manage activities for high-risk psychosocial patients to ensure implementation of multiple services
- Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies
- Identify and assist the team in managing obstacles to treatment and/or discharge plans
- Provide psycho-education for patients and families
- Knowledge of child protection, elder abuse, domestic violence policies and protocols
- Participate in interdisciplinary rounds, case conferences, and staff meetings
- Document in the medical record in accordance with standards set by the Department of Social Work and/or regulatory agencies
- May supervise social work interns
