About the Role
BrightSpring Health Services is seeking a dedicated and compassionate RN Case Manager to join our Hospice team. As a RN Case Manager, you will play a vital role in supporting our patients and their families by providing direct care and coordinating with the interdisciplinary team to ensure an optimal care experience.
Key Responsibilities
- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Administer medication and treatment as prescribed by the physician, and counsel patients and families on meeting their nursing and related needs.
- Maintain accurate and timely clinical notes, and participate in interdisciplinary group meetings to address the medical and emotional needs of patients and families.
- Ensure compliance with all state and federal legal and regulatory requirements, and perform additional responsibilities as assigned.
Requirements
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least 1 year of nursing experience, with 3+ years preferred.
- Experience in hospice or a similar setting is preferred, but not required.
- Knowledge of the hospice philosophy of care and a commitment to clinical and documentation excellence.
