We are seeking a highly skilled and experienced RN Case Manager to join our team at LCMC Health. As a RN Case Manager, you will play a critical role in facilitating and coordinating patient care to ensure quality and efficient outcomes.
Key Responsibilities- Develop and implement individualized care plans that consider patient goals, preferences, and desired level of involvement in the care management plan.
- Assess patient health status through outbound calls and identify gaps or barriers in treatment plans.
- Coordinate care for patients and make referrals to outside sources as needed.
- Ensure patients discharging from hospital or emergency department receive necessary services and resources, including medication reconciliation.
- Triage symptom-based calls per approved protocols for a wide range of patients.
- Enroll patients and manage alerts for various home monitoring programs.
- Provide approved general health information and guidance to patients and caregivers.
- Document all workflows in EPIC system for purposes of tracking and quality assessment.
- Provide patient education to assist with self-management of wellness and disease.
- Form partnerships with providers to achieve quality and cost management objectives.
- Track and monitor enrolled patients throughout the continuum of care.
- Active RN licensure.
- Acute care hospital clinical experience required.
- Knowledge of CMS Conditions of Participation preferred.
- Knowledge of payor rules/regulations preferred.
- Excellent written and verbal communication skills.
- Excellent computer skills.
- Critical Thinking skills.
- Ability to work independently.
- Team Leader capabilities.
