Job Overview

The Phoenix Residence is committed to providing exceptional care and support to adults with disabilities. As a Support Professional II, you will play a vital role in advocating for the individuals we serve, promoting their independence, and enhancing their quality of life.

Key Responsibilities
  • Advocacy and Support: Serve as an advocate for the individuals served by The Phoenix Residence, ensuring their needs are met and their rights are respected.
  • Person-Centered Planning: Develop and implement individualized plans that promote the individuals' goals, aspirations, and well-being.
  • Activities of Daily Living: Assist with daily living tasks, such as personal care, meal preparation, and household management, while promoting independence and self-sufficiency.
  • Communication and Teamwork: Maintain effective communication with colleagues, supervisors, and individuals served, ensuring seamless coordination of services and support.
  • Training and Development: Participate in ongoing training and professional development to enhance skills and knowledge in areas such as person-centered planning, crisis intervention, and medication management.
Requirements
  • Education: High School diploma or equivalency preferred.
  • Age: Must be 18 years of age or older.
  • Experience: One year of experience working with individuals with physical and/or developmental disabilities or a human services-related field, preferred.
  • Skills: Excellent communication skills, verbal and written; ability to coordinate activities and services; ability to read, write, and speak the English language; and ability to use a computer for required documentation and communication purposes.
