We are seeking a skilled Registered Nurse to join our Transitional Care Unit at Transylvania Regional Hospital. As a key member of our team, you will provide exceptional care to patients transitioning from acute care to home or long-term care.
Responsibilities- Assess and monitor patient conditions, implementing care plans and interventions as needed.
- Administer medications, treatments, and therapies with precision and care.
- Collaborate with a multidisciplinary team to coordinate patient care and discharge planning.
- Educate patients and families on post-discharge care and self-management.
- Maintain accurate documentation and adhere to hospital policies and procedures.
- Current licensure as a registered nurse with the North Carolina Board of Nursing.
- Bachelor of Science in Nursing or enrollment in a BSN or advanced degree program.
- One year of hospital nursing experience preferred.
