At Theoria Medical, we are seeking a highly skilled and experienced Medical Director to join our team. As a Medical Director, you will be responsible for providing high-quality, patient-centered care to residents in skilled nursing facilities across the country.
Key Responsibilities:- Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care
- Provide comprehensive primary care services, including diagnosing and treating acute and chronic illnesses
- Supervise and mentor collaborating nurse practitioners and physician assistants
- Participate in Theoria Medical's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives
- Board certification in a relevant medical specialty
- Experience working in skilled nursing facilities or post-acute care settings
- Strong leadership and communication skills
- Ability to work independently and as part of a team
