We are seeking a skilled Case Manager to join our team at Richmond Behavioral Health Authority. The ideal candidate will have experience working with individuals with behavioral health disorders and/or intellectual disabilities, as well as a bachelor's degree in social work or a related field.

  • Job Summary: We are looking for a highly motivated Case Manager to assess client needs, develop service delivery plans, and coordinate services with other agencies.
  • Responsibilities:
  1. Assess client service needs, capabilities, and appropriateness for services.
  2. Develop an individual service plan of care and services appropriate to the evaluation with the client/family consensus.
  3. Makes referrals and linkages to appropriate agencies for services; coordinates client services and treatment with multiple service providers and agencies.
  4. Evaluates the quality of services provided and changes in client's condition and counsels clients as necessary.
  5. Plans and facilitates group sessions with clients participating in community activities and/or to promote recovery and educate clients.
  6. Assists clients with activities of daily living including hygiene/bathing.
  7. Orients and trains newly hired staff; evaluates staff performance as directed.
  8. Serves as liaison to public agencies and provides information regarding Authority programs and services.
  9. Attends inter-disciplinary team meetings to discuss decisions for client plan of care.
  10. Provides crisis intervention and prevention services.
  11. Prepares a variety of reports; prepares and maintains client charts and other records.
  12. Attends meetings, staffings and conferences as they relate to client, staff and program needs.
  13. Attends court hearings; testifies in court.
  14. Assists in curriculum development and resource tools.
