Career Opportunities: We are seeking skilled Medical Providers to join our multi-disciplinary team at the Community Health Center of Middletown in Middletown, Connecticut. As a Medical Provider, you will be part of an innovative practice environment utilizing Advanced Access scheduling, working closely with colleagues to deliver individual and population-based care.

Responsibilities:
  • Deliver high-quality medical services to patients
  • Collaborate with clinical support teams to ensure seamless care delivery
  • Work effectively in an innovative practice environment with flexible scheduling
Requirements: To excel in this role, you must possess strong clinical skills, excellent communication abilities, and a passion for delivering exceptional patient care. Key qualifications include:

MD/DO Requirements:
  • A degree from an accredited medical school and completion of a family practice residency program
  • Board certification within 24 months of employment, valid licensure in Connecticut, and proficiency in bi-lingual English/Spanish
APRN Requirements:
  • A Master's degree in Nursing with a Family Nurse Practitioner specialization and ANA certification as a Family Nurse Practitioner
  • State of Connecticut APRN and RN licensure, with a preference for bi-lingual English/Spanish skills
Physician Assistant Requirements:
  • An accredited Master's degree in Physician Assistant studies
  • Maintenance of board certification, licensure, and professional organization membership
  • Preference for bi-lingual English/Spanish skills
Salary and Benefits: This rewarding position offers an estimated annual salary of $220,000 - $275,000, depending on experience, along with a comprehensive benefits package that includes competitive compensation, opportunities for professional growth, and recognition for outstanding performance.
