We are seeking a highly skilled RN Case Manager to join our travel healthcare team in Portland, Oregon.

Job Summary

Key Responsibilities
  • Manage patient cases, including assessing patient needs, developing care plans, and coordinating treatment
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
  • Maintain accurate and up-to-date patient records, adhering to relevant regulations and standards
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes
Requirements
  • Degree in Nursing (BSN or MSN)
  • Current RN licensure in the state of practice
  • Minimum 1 year experience in case management or a related field
  • Excellent communication, organizational, and problem-solving skills
