Healthcare Manager RN Job Description

At UnitedHealth Group, we believe everyone deserves the opportunity to live their healthiest life. As a Healthcare Manager RN, you will play a critical role in supporting patients with high-risk health conditions to navigate the healthcare system.

Key Responsibilities:
  • Work with providers and clinic staff to identify patients at high risk.
  • Support longitudinal care of patients with chronic care conditions by performing assessments, medication reconciliation, motivational interviewing, and patient education.
  • Support transition of patients with chronic care conditions from inpatient to outpatient settings by performing assessments, medication reconciliation, and establishing contingency plans.
  • Coordinate with providers to establish or update plans of care.
  • Perform accurate and timely documentation in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare accurate and timely reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.
Requirements:
  • Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of experience.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Excellent verbal and written skills.
  • Solid interpersonal skills.
  • Ability to interact productively with individuals and multidisciplinary teams.
  • Proficient computer skills (Word, Excel, Outlook, Teams) to work efficiently with electronic medical records.
  • Excellent organizational and prioritization skills.
Preferred Qualifications:
  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting.
  • Fluent written and verbal skills in English and Spanish.
