- Conduct admissions assessments and documentation for new residents, ensuring a smooth transition to our community.
- Develop and implement acute and long-term care plans or service plans, tailored to each resident's unique needs.
- Monitor and track clinical systems, such as wound care and infection control, under the direction of the director.
- Provide consultation and education to residents and staff on health-related topics, promoting a culture of wellness and prevention.
- Notify physicians and family members of any changes in a resident's health, ensuring timely communication and documentation.
- Respond promptly to emergencies, prioritizing the safety and well-being of our residents.
- Maintain open communication with all departments and staff members, fostering a collaborative and supportive work environment.
- Develop and lead educational meetings for residents and staff, promoting knowledge sharing and skill-building.
- Participate in the Safety and Disaster Plan, ensuring our community is prepared for any situation.
- Ensure confidentiality of residents' and prospective residents' information, upholding the highest standards of professionalism and integrity.
To be successful in this role, you will need:
- Training from an accredited licensed or vocational nursing program.
- 1+ years of experience in senior care or a related field.
- A current Licensed Practical Nurse's License in the state where you will be practicing.
- CPR certification.
- Acceptable proficiency in English to perform job duties, communicate with colleagues, and respond to emergency situations.
- Basic computer skills.
