Job Summary

The Hospice Nursing Assistant will provide personal care and support to patients in their homes, focusing on delivering high-quality care and promoting comfort, dignity, and well-being. This role requires a strong commitment to patient-centered care, excellent communication skills, and the ability to work collaboratively as part of a multidisciplinary team.

Key Responsibilities
  • Deliver personal care and support to patients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as feeding, toileting, and transferring
  • Monitor and report changes in patient condition to the healthcare team
  • Participate in interdisciplinary team conferences and contribute to patient care planning
  • Document patient care and progress in accordance with company policies and procedures
Requirements
  • High school diploma or equivalent required; training or certification in a related field preferred
  • Current certification as a Certified Nursing Assistant (CNA) required
  • Minimum 1 year of experience in home care, hospice, or palliative care preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
