Job SummaryThe Hospice Nursing Assistant will provide personal care and support to patients in their homes, focusing on delivering high-quality care and promoting comfort, dignity, and well-being. This role requires a strong commitment to patient-centered care, excellent communication skills, and the ability to work collaboratively as part of a multidisciplinary team.
Key Responsibilities- Deliver personal care and support to patients, including bathing, dressing, and grooming
- Assist with daily living activities, such as feeding, toileting, and transferring
- Monitor and report changes in patient condition to the healthcare team
- Participate in interdisciplinary team conferences and contribute to patient care planning
- Document patient care and progress in accordance with company policies and procedures
- High school diploma or equivalent required; training or certification in a related field preferred
- Current certification as a Certified Nursing Assistant (CNA) required
- Minimum 1 year of experience in home care, hospice, or palliative care preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
