Key Responsibilities:

  • Provide direct patient care to patients in the ambulatory setting
  • Ensure safe, cost-effective medication therapy
  • Optimize pharmacotherapy plans through disease state management programs
  • Work under collaborative practice agreements as part of an interdisciplinary team
  • Develop and lead quality initiatives to improve patient care
  • Participate in pharmacy-based projects and multidisciplinary committees
  • Precept student pharmacists and pharmacy residents

Requirements:

  • Pharmacy degree from an ACPE-accredited institution
  • Completed a PGY1 pharmacy residency
  • PGY2 residency in Ambulatory Care is highly preferred
  • Pharmacy licensure or eligibility in Indiana
  • High level of clinical practice skills
  • Enthusiasm for teaching
  • Interpersonal skills to interact effectively with students, health professionals, and patients
