Job Overview

At JobRialto, we are seeking a dedicated Clinical Care Specialist to provide exceptional care for adult patients in our Med Surg department. With a focus on patient-centered care, you will be responsible for overseeing the progress of adult patients from initial treatment through recovery.

This role involves close collaboration with doctors, patients, and families to ensure effective care and prompt response to any changes in patient conditions. As a Clinical Care Specialist, you will implement individualized care plans, maintain accurate patient records, and ensure a smooth transition for patients within the hospital.

Key Responsibilities:
  • Evaluate and monitor patient progress and recovery.
  • Identify and respond to sudden or subtle changes in patients' medical conditions.
  • Provide regular updates to doctors, patients, and family members.
  • Initiate treatment, monitor medication doses, and respond to emergencies as needed.
  • Support patient needs throughout recovery, including in ICU settings.
  • Complete required documentation before patient transfers and maintain accurate patient records.
  • Develop and implement effective, personalized care plans for each patient.

Compensation and Benefits
We offer a competitive salary range of $95,000 - $110,000 per year, based on experience, plus a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
Requirements:
  • Associate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN).
  • Minimum of 2 years of RN experience, preferably in Med Surg or related areas.
  • Current RN license in good standing.
  • Basic Life Support (BLS) certification.
  • Experience with EPIC Electronic Medical Record (EMR) system.
  • COVID-19 vaccination required.

PREFERRED QUALIFICATIONS
  • Bachelor of Science in Nursing (BSN) preferred for additional clinical and patient care knowledge.
