Admissions Nurse

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Admission specialist. As an Admissions Nurse, you will play a crucial role in setting the stage for successful patient care, safe transitions, and positive outcomes for patients leaving Inspira Hospitals.

Key Responsibilities:

• Create designated care plans tailored to each patient's needs, making home visits in Cumberland, Gloucester, and Salem counties.
• Perform essential duties, including medication administration, wound care, treatments, and client assessments, while closely monitoring their conditions and documenting your findings.
• Work closely with transitional care teams, discharge planners, and population health to support patients as they transition from Inspira Hospitals to home healthcare, ensuring seamless and safe care with positive outcomes.
• Utilize state-of-the-art touchpad tablets for accurate documentation, ensuring efficient and effective patient care.

Requirements:

• A current NJ RN license and at least one year of recent clinical experience. Prior home care experience or comprehensive medical assessment experience is a plus, but not required.
• Ability to work independently, manage time effectively, and communicate well with patients, families, and healthcare partners.
• Solid computer skills, with prior experience in electronic medical records (EMR) preferred.
• Willingness to travel to assigned cases in your designated territories.
