LPN Care Coordinator

We are seeking a highly skilled LPN Care Coordinator to join our team at Optum. As a Care Coordinator, you will play a critical role in supporting our patients and their families by coordinating care, promoting health, and preventing disease.

• Conduct telephonic outreach to patients, caregivers, and healthcare teams to ensure seamless transitions of care.
• Perform medication reconciliation and proactive outreach to patients to address care gaps and promote health goals.
• Collaborate with patients, families, and healthcare teams to coordinate services and ensure effective care.
• Assist patients with ongoing self-management goal setting and facilitate follow-up appointments.
• Facilitate access to community resources and services.
• Utilize data reports and electronic charts to track patient adherence and identify areas for improvement.
• Maintain strict confidentiality and perform other duties as assigned.

• Graduate of an accredited school of nursing.
• Current Oregon LPN license.
• Current Basic Life Support (BLS) certification or ability to complete within 90 days of hire.

• 3+ years of recent clinical LPN experience.

• Nursing theory and practice to deliver patient care.
• Chronic disease management with established protocols and nursing judgment.
• Principles of health promotion and disease prevention.
• Knowledge of chronic health conditions and their impact.
• Effective communication and customer service skills.
• Ability to work independently and as part of a multidisciplinary team.
• Community resources knowledge and ability to identify complex problems.
• Experience with care coordination processes and motivational coaching.
