The Clinical Pharmacist is a key member of the healthcare team, responsible for ensuring the safe and effective use of medications in a hospital setting.
Key Responsibilities- Provide clinical pharmacy services to patients, including medication therapy management and patient education.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct medication use evaluations and make recommendations for improvement.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest clinical guidelines and research in pharmacy practice.
- Doctor of Pharmacy (PharmD) degree or Bachelor's degree in Pharmacy.
- Current licensure as a Registered Pharmacist (R.Ph) in the State of Ohio.
- Previous hospital experience, preferably in a clinical pharmacy setting.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
