About the Job:

We are seeking a skilled and compassionate Clinical Nurse Specialist to join our team at Advocate Aurora Health.

Job Summary:

The successful candidate will be responsible for managing patient care in various specialty areas, conducting thorough assessments, and developing personalized plans of care. They will also provide direct patient care, collaborate with healthcare professionals, and maintain accurate documentation.

Responsibilities:

  • Assumes responsibility for caseloads consisting of patients in specialty areas, ensuring timely and effective management of patient care.
  • Conducts initial assessment to obtain comprehensive physical, social, and environmental data, informing the development of individualized plans of care.
  • Establishes and implements plans of care in accordance with physician orders and patient needs, adjusting as necessary to ensure optimal outcomes.
  • Provides direct patient care, identifying problems, modifying nursing care plans, and evaluating outcomes within an appropriate timeframe.
  • Collaborates with family members and other healthcare professionals, as needed, to ensure seamless care coordination and communication.
  • Maintains accurate and up-to-date documentation, including visit assessments, OASIS, 485, and verbal orders, adhering to established standards.
  • Communicates effectively with physicians to confirm and update patients' plans of care, providing status updates and facilitating ongoing communication with the healthcare team.
  • Facilitates discharge planning according to established standards, utilizing resources to ensure continuity of care after discharge.
  • Participates in chart audits and continuous quality improvement activities, identifying opportunities for process improvement and contributing to the enhancement of services provided.
  • Maintains accurate clinical and patient records according to agency, state, and federal guidelines, ensuring compliance with regulatory requirements.

Requirements:

  • Registered Nurse license issued by the state in which the team member practices.
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA), obtained within 6 months unless department leader has determined it is not required.
  • A valid driver's license issued by the Division of Motor Vehicles or the state of Illinois.
  • Associate's Degree in Nursing.
  • Typically requires 3 years of experience in clinical nursing.
  • Excellent communication skills, strong interpersonal skills, and the ability to work with diverse clients with patience and respect.
  • Demonstrated ability to work well with physicians and other professionals in a direct and positive manner.
  • Proficiency in clinical skills with the ability to work under direction and make sound judgments.
  • Ability to operate necessary equipment, including FAX, computer, copier, and telephone system.
  • Tolerance for differences and appreciation of multi-culturalism and diversity of patients and their families.
  • Proof of a valid driver's license and a registered, functional car with adequate insurance coverage.

Compensation and Benefits:

The estimated salary range for this position is $80,000 - $120,000 per year, depending on experience and qualifications. Additionally, you will be eligible for a comprehensive benefits package, including health insurance, retirement savings plan, paid time off, and education assistance.
