At Professional Case Management, we are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our team as part-time or full-time healthcare providers. As an LPN with our organization, you will have the opportunity to make a meaningful impact on the lives of our clients, providing high-quality care and support in the comfort of their own homes.

Key Responsibilities:
  • Assess client needs and develop personalized care plans
  • Deliver nursing care and services in accordance with federal, state, and local laws and regulations
  • Collaborate with regional directors, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to clients and their families on health-related topics and disease management
  • Document client progress and maintain accurate records

Requirements:
  • Graduate of a state-approved school of professional nursing
