The Hematology BMT Care Coordinator at TriStar Centennial Medical Center plays a critical role in the care of adult hematologic-oncology patients. This position requires strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment. As a coordinator, you will be responsible for managing patient flow, coordinating care with interdisciplinary teams, and providing education to patients and families.

Key Responsibilities
  • Coordinate patient transfers between units and departments
  • Develop and maintain patient care plans, including medication regimens and treatment schedules
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care transitions
  • Provide education and support to patients and families regarding cancer diagnosis, treatment options, and self-care
Requirements
  • Diploma or associate degree in nursing
  • Registered Nurse license in the state of practice
  • Oncology Nursing Society (ONS) Chemo/Biotherapy Fundamentals Provider or equivalent
  • 1 year of experience in oncology nursing
