Key Responsibilities- Provide Routine Care: Administer medications, take vital signs, and provide general observation of clients' health status.
- Assist RN and Physician: Work under the supervision of a Registered Nurse and collaborate with physicians to ensure clients receive the best possible care.
- Communicate with Clients and Families: Maintain open and effective communication with clients, their families, and guardians to ensure their needs are met.
- Report Changes and Incidents: Accurately document and report any changes in clients' health status or incidents that occur during care.
- Coordinate Admissions and Discharges: Assist with the coordination of client admissions and discharges, ensuring a smooth transition of care.
- Participate in Quality Improvement Activities: Contribute to quarterly quality improvement activities, weekly team meetings, and Interdisciplinary Team meetings to ensure continuous improvement in care delivery.
- Education: Completion of a state-approved training program (e.g., diploma, associate's degree, etc.)
- Qualifications: Must be a Licensed Practical/Vocational Nurse (LPN or LVN) in good standing, with at least 6 months of direct nursing experience.
- Certifications: CPR certification and ability to use a computer utilizing company documentation systems.
- Other: Valid driver's license, with a satisfactory driving record, as defined by ResCare's vehicle policy.
ResCare Community Living has four decades of experience in the disability services field, providing support to individuals who need assistance with daily living due to an intellectual, developmental, or cognitive disability. Our community living services are provided in 27 states with several thousand programs and a comprehensive range of high-quality services, including community living, adult host homes, behavioral/mental health support, in-home pharmacy solutions, telecare, and remote support.
