Job Overview

As a Care Manager at Sunrise, you will be responsible for providing exceptional care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.

Responsibilities & Qualifications
  • Build Meaningful Relationships: Develop strong bonds with seniors and their families, providing assistance with daily activities and getting to know their unique preferences and personalities.
  • Notify Management: Inform management of changes in condition and recommend adjustments in the level of care and service.
  • Assist with Life Skills: Help residents with life enriching activities as indicated on their individual profile.
  • Participate in ISP Development: Collaborate in the development of Individualized Service Plans (ISP) and monthly updates.
  • Responsible for Residents: Ensure the well-being of your designated group of residents during the shift, knowing their whereabouts and checking on them regularly.
  • Partner with Community Team: Work together with the community team to maintain compliance with OSHA requirements and promote Risk Management programs and policies.
  • Provide Excellent Customer Service: Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Qualifications:
  • Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
  • Positive Attitude and Flexibility: Show a positive attitude, flexibility, and ability to work in a team environment.
  • Education and Certifications: Possess a High School diploma/GED, and may require state-specific certifications.
  • Age Requirement: Must be at least 18 years old.
  • Previous Experience: Previous experience working with seniors is preferred.
