Medical Advocate for Community Residences

The Residential Nurse Educator will be responsible for:
- Meeting with new admissions to review medical history and link them with primary care physicians
- Coordinating post-discharge care for clients who have been hospitalized or had surgery
- Attending medical appointments with clients who need assistance communicating with their physicians
- Providing education and training to staff on medical topics
- Facilitating our Health & Safety Committee to review and make recommendations for clients with complex medical needs
The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently and as part of a team.
