At Community Healthcare Partners, we are seeking a skilled Licensed Mental Health Therapist to join our team. As a Licensed Mental Health Therapist, you will have the opportunity to provide mental health services to patients in a variety of settings, including their own homes, assisted living communities, nursing homes, or at our office.

About the Role:
  • Assess and evaluate patients to formulate a personalized plan of care in coordination with the patient's caregiver, healthcare team, and community resources/referrals.
  • Work independently, with the support of a physician and a team of Mental Health Therapists and Psychiatric Mental Health Nurse Practitioners (PMHNP).
  • Provide one-on-one therapy sessions to patients, helping them understand, identify, and set therapeutic goals for concerns such as trauma, mental/personality/behavioral disorder, and family dysfunction.
Key Responsibilities:
  • Develop and implement individualized plans of care for patients.
  • Collaborate with healthcare teams and community resources to ensure comprehensive care.
  • Communicate effectively with patients, caregivers, and healthcare teams through written and verbal communication.
Requirements:
  • Active full license or provisional license as a Licensed Mental Health Therapist (LIMHP) in the state of Nebraska.
  • Two (2) years of experience as a Licensed Mental Health Therapist providing direct psychotherapy services to individuals and families in an outpatient setting.
  • Ability to work with youth, adult, and geriatric populations.
  • Solid computer and basic technology skills to complete clinical documentation and incident reports.
