Key Responsibilities
  • Conduct comprehensive assessments of patient and family needs, identifying outcomes and establishing a plan of care.
  • Provide nursing care to patients and families, incorporating evidence-based practice and considering current statutes, rules, and regulations.
  • Collaborate with patients and families to establish realistic and measurable patient expected outcomes, taking into account age, culture, and capabilities.
  • Coordinate and delegate care effectively, ensuring a safe and effective delivery of services.
Requirements
  • Associate's or Vocational degree in Nursing
  • 0 to 2 years RN experience
  • Preferred: Bachelor's degree in Nursing, homecare/hospice experience, or certifications in Case Management or Palliative Nursing
