Key Responsibilities- Provide safe and effective medication therapy to patients, ensuring compliance with established policies and procedures.
- Monitor and evaluate patient drug regimens, identifying potential contraindications, drug-drug interactions, and other safety concerns.
- Collaborate with healthcare teams to develop and implement individualized patient care plans.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Supervise and direct pharmacy support personnel, ensuring accurate and timely completion of tasks.
- Doctor of Pharmacy degree from an accredited institution.
- Current pharmacy registration in the state of practice.
- State licensure within 60 days of start date.
- BLS or ACLS certification within 180 days of start date.
This is a per diem, as-needed position, with a TBD work schedule.
