We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Aveanna Healthcare. As a key member of our home health care team, you will provide high-quality nursing care to patients in the comfort of their own homes.
Responsibilities- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in and update the nursing care plan to ensure effective patient outcomes.
- Recognize changes in patient needs and responses, implementing care to prevent risk or reduce risk.
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Provide care utilizing infection control measures that protect both staff and patient according to OSHA standards.
- Educate patients and families regarding disease processes, self-care techniques, and prevention strategies.
- Graduate of an accredited school of nursing.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Ability to speak, write, read, and understand English.
- Ability to travel.
- Ability to sufficiently reposition patients and move equipment without assistance.
- Visual and hearing acuity.
