Job Description

Job Summary:

Oakland Heights Nursing and Rehabilitation is seeking a skilled and compassionate Nurse Leader to join our team. As a Charge Nurse, you will play a vital role in providing high-quality patient care and leading our nursing team.

Key Responsibilities:

  • Lead by Example: Demonstrate exceptional critical thinking skills and provide guidance to nursing staff to ensure seamless patient care.
  • Patient Care: Provide safe and accurate medication administration, wound care, and patient assessments.
  • Team Management: Supervise and support Certified Nursing Assistants (CNAs) to ensure efficient and effective care delivery.
  • Communication: Listen to residents and their families, address their concerns, and provide timely updates on patient progress.
  • Documentation: Accurately and thoroughly document patient progress notes and vital signs.
  • Quality Assurance: Monitor and report any changes in patient condition to the nurse supervisor and ensure adherence to facility policies and procedures.

Requirements:

  • Education: Completion of RN training from an accredited school or college.
  • Licensure: Valid nursing license in good standing.
  • Experience: Minimum of 1 year of skilled nursing experience preferred.
  • Skills: Excellent communication and leadership skills, ability to work effectively in a team environment, and proficiency in using computer or tablet to enter resident data.

Physical Requirements:

  • Physical Demands: Ability to move intermittently throughout the day, lift up to 25 pounds, and work in a fast-paced environment.

Essential Duties and Responsibilities:

  • Ensure Patient Confidentiality: Maintain patient confidentiality and provide care in a private and respectful manner.
  • Monitor Vital Signs: Check vital signs, including temperature, pulse, and blood pressure.
  • Provide Wound Care: Dress wounds and provide proper wound care.
  • Insert Catheters: Insert catheters and provide proper care.
  • Listen to Residents: Listen to residents and their families, address their concerns, and provide timely updates on patient progress.
  • Document Patient Progress: Accurately and thoroughly document patient progress notes and vital signs.
  • Report Changes in Patient Condition: Timely report any changes in patient condition to the nurse supervisor.
  • Supervise CNAs: Supervise and support CNAs to ensure efficient and effective care delivery.
  • Ensure Equipment Maintenance: Ensure equipment is in good operating order and report any maintenance needs to the nurse supervisor.
  • Monitor Residents at Risk for Falls: Monitor residents at risk for falls and take necessary precautions to prevent falls.
  • Collect Lab Samples: Collect lab samples and ensure proper handling and storage.
  • Abide by Infection Control Policies: Follow Infection and Control policies to prevent the spread of infection.
  • Report Occupational Exposures: Report any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately.
  • Attend In-Service Training: Attend annual facility in-service training programs to stay up-to-date on the latest policies and procedures.
